DonorPass Saves Time!
Completing a health history questionnaire is an important part of the blood donation process that is required by the Food and Drug Administration (FDA). Use DonorPass to complete the health history questionnaire before arriving at the donation location. DonorPass is accepted at all community donor center and mobile blood drives.
The FDA requires that the online health history questionnaire be completed on the same day as your donation (beginning at 12:01 a.m.).
DonorPass can now successfully be completed using either Windows or a Mac and saved on your smartphone.
Am I able to use a Mac with DonorPass?
Yes, DonorPass is now compatible with Mac computers. When using a Mac we recommend you email your DonorPass instead of printing it before coming to donate.
What are my options with printing or emailing my DonorPass?
If you have a laser printer, you can print your DonorPass. If you don’t have a laser printer, upon completion of the health history questionnaire, you can email your DonorPass to yourself. Please note if you are using a Mac or Safari or Firefox browser, we recommend using the email option, then print your DonorPass from the email or view it on your smartphone. Some cell phone or tablet screen protectors do not allow our scanners to read your completed DonorPass questionnaire. In this case, you will need to re-answer the questions at the donor center unless you have a printed copy with you.
How can I use my smartphone with DonorPass?
After completing the health history questionnaire, enter your email address and we will send you the completed questionnaire. When you present to donate, open the attachment in the email and our staff will scan the information. If you have misplaced the email, you will have to retake the health history questionnaire again before donating. If we are unable to scan your completed DonorPass questionnaire due to a screen protector on your smartphone, you will need to re-answer the questions unless you have a printed copy with you.
What is the best way to print the questionnaire using either FireFox or Safari?
When using FireFox, please reference this quick tutorial. When using Safari, it is best to email your DonorPass to yourself. When you open the email, your DonorPass will be a pdf attachment, click on the attachment and print.
Before starting DonorPass, please make sure your computer has the following:
- Internet Explorer 6 or above
- Firefox 3 or above (email option is recommended; tutorial)
- Google Chrome X
- Safari X (email option is recommended)
- A Laser Printer (if printing)
- Adobe Acrobat Reader (If you have not installed Acrobat reader you will be prompted to do so.)

Remember you MUST bring your DonorPass with you to your donation. If you forget to bring your DonorPass, you will be required to retake the health history questionnaire before donating. When you arrive, staff will scan your DonorPass and review your health history with you.
